시험 개요¶
본 임상시험은 췌십이지장절제술(pancreaticoduodenectomy, PD)을 받는 환자에게 N-butyl-2-cyanoacrylate(Histoacryl®)를 췌장 실질(pancreatic parenchyma)에 주입하여 췌장공장문합술(pancreaticojejunostomy, PJ)의 안전성을 높이고 수술 후 췌장 누출(postoperative pancreatic fistula, POPF) 발생률을 줄이는지 평가합니다. 연구 대상은 90명의 환자이며, 시험군과 표준 췌장공장문합술을 시행하는 대조군으로 무작위 배정합니다. 주요 평가지표는 국제췌장수술연구그룹(ISGPS) 정의에 따른 POPF 발생률입니다.

선정 기준: * 임상 지침에 따라 근치적 치료 목적으로 악성 병변에 대해 췌십이지장절제술을 받는 환자 * 부드러운 췌장 질감 * 작은 주췌관 직경 (<3 mm) * 동의서 획득
제외 기준: * cyanoacrylate 또는 Lipiodol®에 대한 알려진 과민반응 * 극도로 단단하고 섬유화된 췌장 * 남은 췌장을 침범하는 심각한 췌장염 * 수술 부위의 활성 감염 * 조절되지 않는 응고장애 * 심각한 내과적 질환으로 수술에 부적합한 환자 * 복막, 간, 원격 림프절 전이 및 다른 장기 침범 등 원격 전이가 있는 수술 불가능 환자 * 진단적 복강경 검사에서 절제 불가능한 종양 * 좌측, 중앙, 전 췌장절제술 또는 기타 완화적 수술이 필요한 환자 * 임신 중이거나 수유 중인 여성 * 심각한 정신 장애가 있는 환자 * 혈관 침간이 있어 혈관 절제가 필요한 환자 * 연구 참여를 거부한 환자

Inclusion Criteria:
- Patients undergoing pancreaticoduodenectomy for malignant lesions meeting the curative treatment intent in accordance with clinical guidelines.
- Soft pancreatic texture.
- Small main pancreatic duct diameter (\<3 mm).
- Informed consent obtained.
Exclusion Criteria:
- Known hypersensitivity to cyanoacrylate or Lipiodol®.
- Extremely hard, fibrotic pancreas.
- Significant pancreatitis involving the pancreatic remnant.
- Active infection at the surgical site.
- Uncontrolled coagulopathy.
- Unfit patients for surgery due to severe medical illness.
- Inoperable patients with distant metastases, including peritoneal, liver, distant lymph node metastases, and involvement of other organs.
- Irresectable tumors in diagnostic laparoscopy.
- Patients requiring left, central or total pancreatectomy or other palliative surgery.
- Pregnant or breastfeeding women.
- Patients with serious mental disorders.
- Patients with vascular invasion and requiring vascular resection.
- Patients refused to participate in the study.
